When plans go awry, the result can lead to unexpected consequences. Sometimes, one little action can make all the difference; and in some of those situations, things can quickly become fatal. The cause is frequently celebrated, but the effect is rarely discussed. No one likes to talk about the aftermath. Often, others involved dissipate just as quickly as the aftereffect.
