More Description
OUR GOAL IS TO INSPIRE INDIVIDUALS TO TAKE CHARGE OF THEIR HEALTH. We have over twenty years of experience in working with retirees to improve their health outlook. Our epidemiologists have developed health outcome software that was key in providing meaningful feedback so individuals can, with their physician, make progress in their health status. The purchase of the paperback version includes a copy of our "Field Kit" software that was used in the CDC Wellness program. IT IS MODIFIED TO SERVE AS YOUR OWN PERSONAL HEALTH OUTCOME MONITORING TOOL. Also included are other resources that will be helpful in establishing a personal program. Contact us for details.
The CDC originally funded HealthLink Wellness as a program committed to reducing the risk of chronic disease by promoting health screening, health education and exercise. Its strategy has been to develop partnerships that engage the individual through social networking in conjunction with their primary care physician. Essential to the health communication process was the use of scientifically derived outcome measures as a means of monitoring both individual and total group health status.
The HealthLink Wellness program was originally funded to promote the health for our nation's seniors. Its analytical outcome measures are just as valid for all adults. After the CDC funding phase, we have since assisted other community and employer based organizations in conducting health fairs that included a broad range of participant ages. With a broader age distribution, the potential of reaching younger adults further enhances the ability for prevention and early detection, the gold standard for maintaining population wellness.
One outcome measure used since the inception of the program is an estimate of the ten year probability of coronary heart disease originally developed by the Framingham Heart Study. Much of the now common knowledge concerning heart disease, such as the effects of diet, exercise, and common medications such as aspirin, is based on this longitudinal study. Since the Framingham study based index has the endorsement of both the American College of Cardiology and American Heart Association, it was readily accepted by our physician partners.
The CDC grants were used to determine the feasibility of coordinating our community efforts with those of the primary care physician, creating an environment where the patient, community, and medical office work as a team.
