Contact us for details.The CDC originally funded HealthLink Wellness as a program committed to reducing the risk of chronic disease by promoting health screening, health education and exercise. Its strategy has been to develop partnerships that engage the individual through social networking in conjunction with their primary care physician. Essential to the health communication process was the use of scientifically derived outcome measures as a means of monitoring both individual and total group health status.The HealthLink Wellness program was originally funded to promote the health for our nation's seniors. Its analytical outcome measures are valid for all adults. After the CDC funding phase, we have since assisted other community based organizations in conducting health fairs that included a broad range of participant ages. With a broader age distribution, the potential of reaching younger adults increased which further enhanced the ability for prevention and early detection, the gold standard for maintaining population wellness.One outcome measure used since the inception of the program is an estimate of the ten year probability of coronary heart disease originally developed by the Framingham Heart Study. Much of the now common knowledge concerning heart disease, such as the effects of diet, exercise, and common medications such as aspirin, is based on this longitudinal study. Since the Framingham study based index has the endorsement of both the American College of Cardiology and American Heart Association, it was readily accepted by our physician partners.The CDC grants were used to determine the feasibility of coordinating our community efforts with those of the primary care physician, creating an environment where the patient, community, and medical office work as a team.
