This Comfort Assessment Workbook is Step 1 in PalliPartners' Community-Based Palliative Care Workbook Series, a comfort resource tool available through www.PalliPartners.com. If you are managing a serious or chronic illness at home, this Series of three Workbooks offers a systematic approach to help you first, understand and record the physical, mental, emotional and/or spiritual symptoms you're experiencing as a result of your illness and then, create your own palliative care plan and assemble your own palliative care team as means of being as comfortable as possible. You are encouraged to present your doctor with your completed plan and team for his/her approval and participation. Please be sure to look for Steps 2 (Linking Resources Workbook) and 3 (Create My Plan/Assemble My Team Workbook). Completing the Series, along with accessing additional resources available at www.PalliPartners.com, will empower you to locate, right in your own community, the products and services you need to be comfortable at home.
